Creatine: The One Supplement You’re Probably Not Taking (But Should Be)

It’s cheap. It’s safe. It works. So why are so many people still skipping it?

Walk into any supplement store, and you’ll find shelves packed with powders, pills, and potions promising miraculous results — each one more expensive than the last. Here’s the irony: the most well-researched, most universally recommended performance supplement costs about twenty cents a day.

Creatine monohydrate. Bro-science staple turned mainstream powerhouse. Peter Attia — one of the most evidence-obsessed physicians in the longevity space — says it’s one of the very few supplements he recommends to nearly every patient who exercises. Layne Norton, PhD, puts it even more bluntly. He tells people: “I don’t even know why we’re having this conversation anymore.”

That’s how settled the science is. So let’s cover everything you need to know.

What Is Creatine?

Creatine is a naturally occurring compound your body already makes — primarily in your liver, kidneys, and pancreas. You also get small amounts from eating meat and fish. The problem? Your muscles can only store so much of it, and most people’s natural levels are well below what’s optimal for performance and health.

When you supplement with creatine monohydrate, it travels into your muscle tissue and gets stored as phosphocreatine — essentially a high-powered energy reserve. Think of it as a backup battery in your cells.

Quick note on form

The research is overwhelmingly done on creatine monohydrate. Despite what supplement labels might suggest, there’s no compelling evidence that fancier, more expensive forms (creatine HCl, buffered creatine, etc.) work any better. Buy the plain stuff. It’s cheap, it works, and it’s what the studies actually test.

How Does It Work?

To understand creatine, you need a thirty-second crash course on cellular energy.

Your muscles run on a molecule called ATP (adenosine triphosphate) — the body’s energy currency. When a muscle contracts, ATP loses a phosphate molecule and becomes ADP (adenosine diphosphate). That ADP is temporarily “spent” — it needs to get recharged before it can power another contraction.

This is where phosphocreatine steps in. It donates its phosphate group back to ADP, regenerating ATP almost instantly. The result: your muscles can sustain high-intensity effort for longer before fatigue hits.

The analogy

Think of ATP as a charged battery. When it powers your muscles, it drains. Phosphocreatine is the charger — and creatine supplementation means you have more chargers available, so your batteries stay topped up longer during intense effort.

There’s also a second mechanism at work: creatine is a powerful osmolyte, meaning it pulls water into muscle cells. This cell volumization — the muscles being more hydrated — appears to be anabolic in its own right. And before you say “that’s just water weight,” keep in mind: muscle is approximately 70% water. More hydrated muscle cells appear to function better, recover faster, and may even contract more efficiently.

What Does It Actually Do for You?

Strength and Power

This is the most well-established benefit. Dozens of randomized controlled trials — some running over a year long — consistently show that creatine supplementation increases strength, power output, and training performance. A 2024 meta-analysis found that creatine combined with resistance training produced significant improvements in one-rep max across a wide range of populations, including older adults.

Muscle Mass

Creatine users consistently see greater gains in lean mass compared to those not supplementing. Some of this is water (as discussed above), and some is actual added muscle tissue from being able to train harder, recover better, and trigger more protein synthesis over time. Either way, the size and strength gains are real and repeatable.

Brain Health and Cognition

For the first two decades of creatine research, scientists were almost exclusively focused on muscle. That made sense — the performance benefits were obvious and repeatable. But over the last five to ten years, a growing body of research has turned its attention to what creatine does inside your skull, and the findings are genuinely interesting.

Here’s the foundation: your brain accounts for only about 2% of your body weight but burns roughly 20% of your total daily energy. It is one of the most metabolically demanding organs in the body, running almost entirely on ATP. The same phosphocreatine system that buffers energy in your muscles operates in your brain cells too — which means creatine has a plausible, well-understood mechanism for supporting cognitive function, not just a vague supplement-label claim.

The research has zeroed in on a few specific areas where the evidence is most compelling:

Memory

Multiple studies and reviews indicate improvements in short-term memory and working memory with creatine supplementation. A 2025 meta-analysis published in Frontiers in Nutrition found small but statistically significant improvements in short-term memory and information processing speed across multiple randomized controlled trials. The effects are most consistent — and most meaningful — in older adults, whose brain energy metabolism naturally declines with age, and in people with lower baseline creatine levels (vegetarians, vegans, and light meat eaters).

Mental Fatigue and Sleep Deprivation

This is where some of the most striking research lives. When your brain is under metabolic stress — from sleep deprivation, prolonged mental effort, or low oxygen — its energy reserves get depleted fast. Creatine appears to act as a buffer, helping the brain maintain ATP availability when it’s under pressure.

A 2024 study published in Nature Scientific Reports found that a single dose of creatine improved cognitive performance and measurably altered high-energy phosphate levels in the brain during sleep deprivation. Subjects who received creatine outperformed the placebo group on tasks requiring logical reasoning, processing speed, and sustained attention — even after a full night without sleep. A follow-up study confirmed the effect at lower doses, finding that creatine mitigated the cognitive decline associated with sleep deprivation across multiple cognitive tests.

For anyone who has had a rough night’s sleep before a demanding day — which covers basically every adult alive — that’s a meaningful finding.

Aging and Long-Term Brain Health

As we age, brain creatine levels naturally decline. At the same time, the risk for cognitive impairment increases. Researchers are now asking whether chronically supporting brain energy metabolism with creatine supplementation could slow that trajectory. The data here is still emerging — this is an active area of investigation — but the mechanistic rationale is solid, and the early signals are promising enough that several longevity-focused clinicians, including Peter Attia, have started recommending creatine in part for its potential brain benefits, particularly in older adults and those who eat little to no meat.

Bone and Aging

Emerging research also points toward benefits for bone density and fall prevention in older adults — particularly when combined with resistance training. This isn’t the primary reason most people take creatine, but it adds to the case for its inclusion in the longevity toolkit.

Is It Safe?

Yes. Emphatically, yes — and the evidence base here is unusually strong for a supplement.

Creatine monohydrate has been studied extensively for over 30 years with no evidence of harm in healthy individuals. The concern you’ll occasionally hear about kidney damage is based on a misunderstanding: creatine supplementation does raise your blood creatinine levels, and creatinine is sometimes used as a marker of kidney function. But the presence of elevated creatinine doesn’t mean kidney damage is occurring — it just reflects increased creatine turnover. It’s a marker issue, not an organ issue.

A note for those with existing kidney issues

If you have existing kidney disease or compromised kidney function, talk to your doctor before starting creatine. For healthy people, the research is detailed — it’s not a kidney risk. But if your kidneys are already under stress, it’s worth a conversation with your physician first.

What about hair loss? There’s one 2009 study that showed an increase in DHT (a hormone linked to male pattern hair loss) with creatine use. This single study has never been replicated. No randomized controlled trial has shown that creatine causes actual hair follicle changes or hair loss. As Layne Norton notes, the data on this concern simply isn’t compelling.

Who Should Be Cautious?

Creatine is appropriate for most healthy adults. That said, there are a few situations to be aware of:

Existing kidney disease: Consult your physician before starting.
Certain rare metabolic conditions: Some rare genetic disorders affect how the body processes creatine. These are uncommon, but worth noting.
Pregnancy and breastfeeding: The research here is simply insufficient. Avoid unless directed by your OB/GYN.
Beyond these situations, creatine has an unusually clean safety profile for a supplement with this level of effectiveness.

How to Take It

The Dose

Creatine is the most effective ergogenic aid ever studied. The research-supported standard dose is 3–5 grams per day — consistent, no drama required.

For younger athletes focused purely on strength and hypertrophy, 5g daily is the well-established standard and fully sufficient to saturate muscle tissue over three to four weeks.

But if you’re 40-plus and your goals include muscle, bone, and brain health — which describes most Ascend athletes — emerging research suggests a target closer to 0.10–0.14 g/kg of bodyweight per day may be more appropriate. For most people, that lands somewhere between 8 and 11 grams daily. The logic: skeletal muscle saturates relatively quickly at lower doses, but bone and brain tissue are far slower to absorb creatine and likely require higher chronic exposure to see meaningful benefit. Split that dose across two servings with meals, and most people handle it without any GI issues.

Quick math

At 0.14 g/kg/day, a 175 lb (80 kg) athlete needs about 11 grams per day. Two 5g servings — morning and evening with meals — gets you there cleanly.

Loading vs. Just Starting

You may have heard of “loading” creatine — taking 20–25g per day for a week before dropping to maintenance. This does saturate your muscles faster (roughly 1 week vs. 3 weeks). But the trade-off is significant GI discomfort for many people. Since we’re playing the long game here, there’s no real advantage to loading. Take 5g a day, be patient, and in three to four weeks you’ll be fully saturated — same endpoint.

Timing

Honestly? It doesn’t matter much. There’s a small amount of evidence suggesting taking it post-workout may be marginally better, but the effect is minimal. The most important thing is consistency. Take it at the same time every day — whatever fits your routine. Layne Norton takes his first thing in the morning. Many people mix it in their protein shake. The best timing is the one you’ll actually stick to.

Training Days vs. Rest Days

Take it every day, not just on training days. The benefits of creatine come from keeping your muscles fully saturated. Skipping rest days just makes that harder to maintain and easier to forget. It’s cheap enough that there’s no reason to go on and off based on your workout schedule.

Do You Need to Cycle Off?

No. There’s no evidence you need to cycle on and off creatine. While your body slightly reduces its endogenous production while you’re supplementing, intramuscular creatine levels remain elevated, which is the goal. If you do stop taking it, you’ll lose the benefits within about a month. There’s no hormonal feedback loop here (creatine is not a hormone), so there’s no physiological need to take breaks the way you might with other compounds.

The Bottom Line

In a supplement industry overflowing with overpriced products, weak evidence, and marketing hype, creatine monohydrate stands almost completely alone as the thing that everyone agrees on. The researchers agree. The clinicians agree. The coaches agree.

It’s safe. It’s inexpensive. It works for strength and muscle mass across every age group studied. It likely supports brain health, which matters more as we get older. And the only thing stopping most people from taking it is that nobody’s paying for a Super Bowl commercial to tell you about an unglamorous bag of white powder that costs less than your morning coffee.
